What this website collects
The consultation form requires first name, email address, and service of interest. It also requires a general location for physical therapy or coaching. Last name, phone number, contact preference, call format, best contact time, and a general question are optional. The Mind Over Nerves waitlist collects first name and email address. No account is created. This policy will be updated before account, payment, or community-member data is collected.
Health information
These forms are not intended for health or medical information. Do not submit diagnoses, symptoms, medications, medical records, insurance information, medical history, or other health details. Regular email is not a secure place to send private medical or health information.
Inquiry email and optional updates
An email address is required on the consultation form so Mahad can respond to the inquiry. A consultation request does not add you to marketing messages. The separate “Email me occasional MS resources and Mind Over Nerves updates” checkbox is unchecked by default. Only people who select it are added to the updates list. The standalone waitlist form is also an express request to receive those updates.
How information is used and stored
Consultation details are used to respond to the inquiry and keep appropriate business records. Waitlist details are used to send requested resource and Mind Over Nerves updates. The consultation record also stores whether that separate option was selected and, when selected, the consent time. Entries are kept in a private Lovable Cloud database and are not publicly readable. Mahad MS PT does not sell form information or share it for advertising.
Service providers
Lovable provides website hosting, the private database, and built-in visitor analytics. Google provides Google Analytics 4 website measurement. The site includes a Resend transactional-email connection for form notifications and acknowledgements; delivery depends on that connection being active and a send succeeding. These providers may process the limited information needed to deliver their services.
Analytics
Built-in Lovable visitor analytics may measure visitors, page views, visit duration, bounce rate, traffic source, pages viewed, device category, and country inferred from the browser time zone. This site also uses Google Analytics 4, which sets cookies or similar identifiers and may measure page views, pages and links visited, approximate location, device and browser details, and referring source. Form entries are never sent to Google Analytics, and no name, email address, phone number, message text, or health information is shared with it. This is used to understand overall site traffic, not to build advertising profiles. No advertising pixels, retargeting, or session replay are used.
Retention
Consultation information is kept only as long as reasonably needed to respond to the inquiry and maintain appropriate business records. Waitlist information is kept until you unsubscribe or ask for deletion, unless a longer period is reasonably required for legal or operational records.
