Mahad PT · In-home physical therapy · Northern Virginia
Physical therapy at home for older adults in Northern Virginia.
One-on-one physical therapy in your apartment or home. No clinic trip, no waiting room, no rotating staff. Focused work on the movements that keep you safe and independent.
Please do not send detailed medical or health information by regular email.

One physical therapist. Every visit.
You will never get a rotating cast of aides and assistants. The person who evaluates you is the person who treats you, and he already knows your home, your history, and your goals.
Who this is for.
If any of these sound familiar, in-home physical therapy may be the right next step.
- Falls or a growing fear of falling
- Balance problems or unsteadiness
- Walking difficulty, inside or outside the home
- Leg weakness or legs that tire quickly
- Trouble with stairs
- Difficulty getting up from a chair, sofa, bed, or toilet
- Deconditioning after an illness or hospital stay
- Wanting to stay independent at home
- Arthritis-related stiffness and aching joints
- Neurologic conditions affecting movement, such as Parkinson's, MS, or stroke recovery
What the work focuses on.
Every plan is individual. These are common areas of work, not promised outcomes.
Balance and fall prevention
Steadier footing for the moments that matter: hallways, bathrooms, curbs, and stairs.
Walking safely
Gait, endurance, and confidence on the surfaces you actually walk every day.
Strength for daily life
Sit-to-stand, carrying groceries, household tasks. Strength that shows up in your routine.
Easier transfers
Bed, chair, car, and bathroom transfers practiced until they feel routine, not risky.
Stairs
A step-by-step approach to the staircase in your own home, with the railings you actually use.
Rebuilding stamina
A gradual return to activity after illness, surgery, or a hospital stay, paced to your body.
How payment works.
A referral from your doctor is required before treatment begins. You can still reach out first: Mahad will explain exactly what is needed and help you get started in the right order.
This page is built around the Medicare pathway. If you have Original Medicare and need covered physical therapy, services are billed through Medicare, not private pay.
How it works.
Simple, with no surprises about what comes next.
Step 1
Reach out
Call or text Mahad, or request a consultation online. Tell him who the therapy is for and where you live.
Step 2
Confirm what is needed
Mahad confirms the service area, the location, scheduling, and anything required before treatment begins, including the doctor's referral.
Step 3
Therapy comes to you
It starts with a 60-minute evaluation in your home, then one-on-one visits with the same physical therapist every time.
Why Mahad.
Mahad Ali is a Virginia-licensed physical therapist (license no. 2305217964). His specialty is neurologic mobility conditions, including MS and Parkinson's, which means the complex cases other practices find difficult are his home ground.
He also lives with MS himself, so he understands what it is like to work hard for every bit of independence. That perspective shapes how he treats: patiently, honestly, and always around your real life.
And because this is one independent practice, not a large agency, you work with one person from the first evaluation to the last visit.
For senior living communities.
A resource for your residents, not a sales pitch to your building.
Mahad offers free fall-prevention talks for residents of senior communities across Northern Virginia: practical, plain-language sessions on staying steady at home.
- No cost and no contract to the building
- Residents choose freely; never an endorsement
- One point of contact for your team
- Serving Alexandria, Springfield, Annandale, Burke, Fairfax, Falls Church, and Arlington
For physicians and referral sources.
A reliable place to send patients who cannot easily reach a clinic.
A referral from an authorized healthcare provider is required before physical therapy treatment begins. You may request a consultation before obtaining the referral, and Mahad will explain the next step.
You receive communication back: evaluation findings, the plan of care, and progress updates, so the patient never disappears into a black box. Neurologic presentations, including Parkinson's and MS, are a particular strength. Learn more about the MS specialty focus.
Common questions
Do I need a doctor's referral?
A referral from an authorized healthcare provider is required before physical therapy treatment begins. You may request a consultation before obtaining the referral, and Mahad will explain the next step.
Does Medicare cover this?
Mahad PT is currently enrolling with Original Medicare. Once enrollment is active, eligible covered physical therapy services will be billed through Medicare according to Medicare requirements. If you have Original Medicare and need covered physical therapy, services are billed through Medicare, not private pay.
Is this home health?
No. This is mobile outpatient physical therapy delivered where you live: one-on-one treatment with a licensed physical therapist, in your apartment or home. Home health is a different Medicare benefit with different rules. If you are unsure which one you need, ask during your consultation.
What happens at the first visit?
The first visit is a 60-minute evaluation in your home. We talk about what has changed, what feels unsafe or difficult, and what you want to be able to do. Mahad assesses walking, balance, strength, transfers, and how your home setup affects your movement. You leave with a clear plan.
Where do you treat?
In your apartment or home anywhere in the core service area: Alexandria, Springfield, Annandale, Burke, Fairfax, Falls Church, and Arlington. Treatment happens in your real environment, which is exactly where safer movement matters most.
What if my parent has Parkinson's or had a stroke?
Neurologic mobility conditions are Mahad's specialty area, including Parkinson's, MS, and stroke recovery. Many older adults he works with are dealing with exactly these conditions on top of the usual aging-related changes.
How is this different from going to a clinic?
There is no trip across town, no waiting room, and no being handed off between staff. Every visit is one-on-one with the same physical therapist, in the home where you actually need to move safely.
